Athlete Intake Form

Each athlete is an ambassador for their sport, Sask Sport, and the CSCS. In completing this athlete agreement, the athlete acknowledges and understands the following:
1. The athlete agrees to be considerate of others, including Canadian Sport Centre Saskatchewan partners, participants, volunteers, and staff while agreeing to follow organizational/business policy and procedures as outlined by each facility.
2. The athlete will avoid any conduct or action that disparages Canadian Sport Centre Saskatchewan.
3. The athlete will avoid the use of banned substances and practices that contravene the rules of World Anti-Doping Agency (WADA), the rules of their International Federation or the current Canadian Policy Against Doping in Sport. If an athlete becomes sanctioned by any of the parties CSCS reserves the right to suspend involvement and services as provided by the CSCS and/or our partners.
4. The athlete will act in accordance with the policies as presented on the CSCS website, including but not limited to the Universal Code of Conduct to prevent and address maltreatment in sport (UCCMS), Code of Conduct, Dispute resolution, Conflict of Interest, etc. The policies can be found here for your review prior to signing below.
5. Canadian Sport Centre Saskatchewan cards are to be used solely by the registered athlete to whom the card was issued. Sharing of these cards may result in the athletes card and access being revoked.
6. An updated and valid Canadian Sport Centre Saskatchewan card must be presented every time the athlete wishes to gain access to benefits, programs, and services.
7. It is the athlete’s responsibility to notify Canadian Sport Centre Saskatchewan of any change in personal information including name, AAP carding status, address and other information.
8. Notify Canadian Sport Centre Saskatchewan as soon as possible when unable to attend an event for which you have registered or confirmed attendance.

GymFit Specific:
1. GymFit facilities will not be held responsible for property left in lockers or on the premises.
2. The athlete must treat the gym equipment with care, and respect hours of access and age restrictions for each GymFit facility. It is required that prior to attending a facility the athlete call to understand the available hours of access and any additional access requirements outlined.
3. Participation in sport, general fitness and physical activity programs involves a risk of normal injuries associated with participation. An athlete’s participation in GymFit will be deemed to indicate acceptance of such risks and an understanding that GymFit facilities associated with Canadian Sport Centre Saskatchewan, Sport Medicine and Science Council of Saskatchewan and Sask Sport accept no responsibility for activity related risks.
4. The athlete agrees to abide by the rules of conduct for each GymFit facility.

If the athlete does not abide by the rules outlined in the athlete agreement all privileges offered by Canadian Sport Centre Saskatchewan to the athlete may be terminated.


Privacy Act Information
Canadian Sport Centre Saskatchewan will not share or distribute any of this personal information to any other individual or organization outside of the Canadian Sport Centre Saskatchewan Centres without written consent by the athlete. The information you provide will become part of the Canadian Sport Centre Saskatchewan database to be used for Canadian Sport Centre Saskatchewan funds development initiatives, athlete program initiatives, communication between athlete families and Canadian Sport Centre Saskatchewan, and promotional opportunities. Although your information will be available to all Canadian Sport Centre Saskatchewan Centres, please note that only Canadian Sport Centre Saskatchewan affiliate will ever contact you. If you do not consent to such collection, use or disclosure, or if your information may need updating, please contact your Centre in writing.

-I acknowledge my responsibility to abstain from the use of banned substances as outlined by the current Canadian Policy Against Doping in Sport.

-I give permission for Canadian Sport Centre Saskatchewan staff to share non-specific service usage information amongst themselves and other integrated support team (IST) service providers.

-I give permission for Canadian Sport Centre Saskatchewan to share my permanent address with the Canadian Olympic Committee, Canadian Paralympic Committee, Sport Canada, Coaches Association of Canada and Sask Sport.

-I give permission for Canadian Sport Centre Saskatchewan to use photos and/or videos that I appear in to be used for promotional purposes.

-I give permission to be contacted by electronic messaging regarding Canadian Sport Centre Saskatchewan funds development initiatives, athlete benefits, programs, services, newsletters, performance points and promotional opportunities.
